Make a Referral

To refer a case for CareAssist care management or ClinAssist Forensic Review or claims negotiation you can download a referral form and fax it to us on the number listed on the form. A care management or claims audit specialist will contact you to review the data and answer any questions about the process you may have.

You can also make a referral by calling us toll-free at 1-877-631-9080 and selecting option 1.
